Navigating insurance for physical therapy doesn't have to be confusing. We verify your benefits before your first visit, explain your costs upfront, and handle the billing process so you can focus on getting better.

We accept most major insurance plans and offer straightforward self-pay options for patients without insurance or those who prefer to pay out of pocket.

What to Expect With Insurance

Before Your

First Visit

Call us with your insurance card information and we'll verify your benefits — including whether we are in-network for your plan, your deductible status, your co-pay or co-insurance amount, and whether a referral or prior authorization is required.

We do this proactively so you know exactly what to expect before you walk through the door.

At Your First Appointment

Bring your insurance card so we can make a copy of your insurance card. If your plan requires a referral, We will then walk you through your insurance benefits and confirm the expected cost to you for your appointments.

We will collect any required co-pay, coinsurance, or deductible at the time of service.

Billing & Explanation of Benefits

We submit claims directly to your insurance on your behalf. After your insurer processes the claim, you will receive an Explanation of Benefits (EOB) from your insurance company — this is not a bill. Any remaining patient responsibility will be billed to you separately.

Self-Pay Options

No insurance? High deductible? Prefer simplicity? Our flat-rate self-pay options are straightforward, competitive, and come with no surprise bills.

Many patients with high-deductible plans find self-pay is more cost-effective than paying toward their deductible at the insurance-adjusted rate. We're happy to help you compare both options before your first visit.

We also offer prepaid savings packages: Purchase $500 in clinic credit for $475 (5% savings) or $1,000 in clinic credit for $900 (10% savings). The credit may be used toward eligible services and can be shared among family members.

HSA & FSA

Physical therapy is an HSA (Health Savings Account) and FSA (Flexible Spending Account) eligible expense. Many of our other services — including massage therapy when part of a treatment plan — may also qualify.

We accept HSA and FSA cards directly at all three locations. If you're unsure whether a specific service qualifies under your plan, check with your HSA/FSA administrator.

Common Questions, Straight Answers

Do I need a referral from my doctor to start physical therapy?

No — Wisconsin is a direct-access state. You can begin physical therapy without a physician referral. However, some insurance plans require a referral for coverage purposes. We'll confirm whether your specific plan requires one when we verify your benefits.

How do I know if you're in-network with my insurance?

Call us at (608) 351-3049 (Onalaska / Unity) or (608) 668-6700 (State Road) with your insurance card and we'll check your specific plan before your first visit.

What is a co-pay vs co-insurance?

A co-pay is a fixed amount you pay per visit (e.g. $30 per visit). Co-insurance is a percentage of the allowed amount you pay after meeting your deductible (e.g. 20% of the visit cost). Your EOB and benefits verification will clarify which applies to your plan.

What if my insurance denies a claim?

We will work with you to appeal denied claims where appropriate. If a claim is denied, contact our billing team and we'll review the denial and advise on next steps. You are never on your own in navigating this.

Does insurance cover massage therapy?

In most cases, massage therapy is not covered by standard health insurance unless it is provided by a licensed PT or PTA as part of a documented treatment plan. Our integrative massage services are available at competitive self-pay rates. HSA/FSA may apply in some circumstances — check with your plan administrator.

Does insurance cover personal training or wellness classes?

Personal training and wellness classes are generally not covered by health insurance and are available at self-pay rates. Some HSA/FSA plans may cover exercise programming when prescribed for a specific medical condition — check with your plan administrator.
